Post-earthquake Service Provision Analysis in Sisakht, Yasuj, in 2020(مقاله پژوهشی وزارت بهداشت)

INTRODUCTION: An earthquake is a major natural disaster with widespread and great effects on human life and nature. Taking care of the basic needs of earthquake victims is an essential step that should be addressed immediately. Moreover, the evaluation of various problems after disasters, especially earthquakes, is of crucial importance for the affected people. The management of humanitarian aid and public assistance among these affected people is considered one of the most important post-earthquake concerns. Therefore, the present study was conducted to evaluate the post-earthquake challenges and strengths of providing humanitarian aid services in Sisakht, Yasuj, in February 2019. METHODS: This qualitative case study was conducted from March 2019 to July 2020. The studied population consisted of 20 participants including medical staff (n=10), clergy (n=2), crisis managers (n=4), and military personnel (n=4). The required data were collected through observation and semi-structured interviews, which continued until data saturation. To evaluate the reliability of the data, the strategies recommended by Guba were used. The data were analyzed by conventional content analysis based on the method proposed by Graneheim and Lundman. FINDINGS: The advantages and strengths of this study included humanitarian cooperation and coordination, as well as the timely presence of relief and rescue teams and law enforcement agencies from the post-earthquake first hours, which had a great impact on reducing the suffering of people. The challenges in providing post-earthquake services included health, medical, management, geographical, and spatial concerns and issues related to the earthquake victims (relief goods distribution problem and inappropriate information dissemination as the main challenges). CONCLUSION: Effective and appropriate management policies regarding the management of providing post-earthquake services and elimination of the structural and managerial obstacles can improve the operation and management of humanitarian aid.

Assessment and Optimization of a Spatial Model of Access to Family Physician Centers in Shiraz Using Geographic Information System(مقاله پژوهشی وزارت بهداشت)

INTRODUCTION: The present study aimed to assess and optimize the spatial model of access to family physician centers in Shiraz using the Geographic Information System (GIS Software). METHODS: This descriptive-analytical cross-sectional study was performed in four stages in all family physician centers and clinics in Shiraz. In the first stage, a review study was performed to determine site selection criteria, and in the second stage, the criteria were prioritized and weighted using the method of Analytical Hierarchy Process (AHP). In the third stage, the data were recorded in GIS Software, and finally, in the last stage, the spatial data were assigned to databases. FINDINGS: Based on the results, the field of geography with a weight of 0.549 was the most important area, and the criterion of population age structure with a weight of 0.451 assumed the most critical importance in the selection of appropriate sites for the establishment of family physician centers. According to Moran’s index (MI = 0.055), family physicians are randomly distributed in Shiraz, and about 20% of the population do not have standard access to family physician centers. CONCLUSION: As evidenced by the results of the current study, people living in the suburbs of Shiraz had far less access to family physician centers, as compared to those residing in central parts. Moreover, the spatial distribution of family physicians in Shiraz does not follow a specific pattern. It is necessary to plan and take effective measures to establish new centers or redistribute existing centers in accordance with the priorities obtained from the results of this study in an attempt to increase healthcare access in suburban areas using GIS and other related techniques.

The Effectiveness of Psychological Interventions in the Well-Being of Adolescents Surviving Natural Disasters in East Azerbaijan Province(مقاله پژوهشی وزارت بهداشت)

INTRODUCTION: Given that Iran is among the top five earthquake-prone countries in the world and no region of the country is safe from unforeseen crises, raising awareness regarding the victims' mental health issues is one of the top priorities limiting the extent of problems and subsequent consequences. The present study aimed to investigate the effectiveness of psychological interventions in the well-being of adolescents surviving natural disasters as opposed to regular individuals. METHODS: The study population in the present cross-sectional comparative study consists of high school students (13–18 years old) from the earthquake-stricken village of Sarand in East Azerbaijan Province, and normal students from Zarnaq in the same province. They were assessed through the total population sampling method. The data collection tool utilized was a questionnaire (Ryff) to assess anxiety and depression. Data is analyzed and screened by measures of central tendency (mean and standard deviation) for the first section of variables. In the second section, the analysis of variance (ANOVA) was employed to test the research questions. FINDINGS: The earthquake group achieved the lowest and highest mean scores in the aspects of environmental mastery (9.46), personal growth (10.93), and purpose in life (10.90), respectively. In the Non-Affected group, the lowest mean score is associated with paranoia (2.25) and the highest mean score is linked to anxiety (3.86). The post-test revealed the lowest mean score in the control group was related to autonomy (11.09), and the highest mean score pertained to personal growth (14.38). The scores are normally distributed in all dimensions. Eventually, the obtained data indicated that the psychological interventions performed for the benefit of the disaster-stricken group were significantly more than the group that was not subjected to psychological interventions. However, there was no statistically significant difference between the two groups in terms of sex. CONCLUSION: Providing post-crisis social psychosocial support programs for adolescents consolidates cognitive and behavioral rehabilitation, diminishes stress and vulnerability, enriches relationships with peers, decreases psychosomatic disorders caused by the crisis, and modifies social performance. Moreover, spending time with their peers, family members, and relatives ameliorates various disorders and issues.

Investigation of Traffic Accident Hotspots in Yazd City using GIS(مقاله پژوهشی وزارت بهداشت)

INTRODUCTION: The importance of safety and prevention of traffic accidents have highlighted the necessity of research and investigation in this field. The present study aimed to identify and eliminate the risk factors underlying the occurrence of inner-city traffic accidents. In so doing, scientific and effective solutions can be provided at the lowest cost to implement the slogan of "prevention is better than cure". METHODS: In this study, the accident rate-severity index method was used to identify traffic accident hotspots. The spatial units in which this index (I) was more than twice the average of the total spatial units were identified as Hot Zones FINDINGS: In the present study, the study area (6,600 meters) was divided into 66 100-meter spatial units. Using the accident rate-severity method, 6 (600 m) and 19 (1900 m) spatial units were identified as Hot Zone (9.1%) and Yellow Zone (28.8%), respectively. Finally, 41 Cold Zone spatial units (62.1%) were identified (4,100 m). CONCLUSION: As evidenced by the obtained results, the following measures can be effective in the reduction of road accidents: retrospection of traffic signs in terms of number, size, location, height, installation of speed bumps in the Hot Zone and Yellow Zone spatial units, timely pruning of trees along the road-construction of underpasses and overpasses in some important points of the route (accident hotspots and Yellow Zone). Furthermore, the development of a comprehensive and long-term educational program to improve traffic safety culture in kindergartens and schools can be effective in reducing road accidents.

Sefid Sang Earthquake Measuring 6 on the Richter Scale in Razavi Khorasan Province, Iran, 2017: Challenges and Operations(مقاله پژوهشی وزارت بهداشت)

INTRODUCTION: The present article aimed to study field observations of the 2017 Sefid Sang earthquake, Razavi Khorasan Province, Iran, measuring 6 on the Richter scale with the approach of assessing the behaviors and performing a short analysis on the rescue and relief operations. METHODS: This qualitative study has followed the conceptual analysis approach to research. The sample population was selected with purposive sampling technique from the affected villages of Brashak, Karghash Olya, Drakht Bid, Kelate Menar, Kelate Hajikar, Kharzar, and Chah Mazar to study the behavior and knowledge of the affected people. A goal-based sampling was also applied among the operational managers who were directly engaged in the relief and rescue operations. This research benefited the focus group’s viewpoints. The necessary data were gathered from the answers given to the open questions. The process of research data analysis was in the light of phases proposed by Granheim and Lanman. FINDINGS: The results of this study showed that the disaster preparedness index coefficient among the residents of affected and surrounding villages was low which seriously required enhancement. It was also found out the affected people lacked necessary awareness about general training on the subject of disaster resiliency. Although Red Crescent’s role of disaster response in the context of implementation had been effective, it was found that its other roles of advocacy and support culd be more effective than its implementation role.

Hazard Assessment in Fars Province Hospitals in Iran in 2017(مقاله پژوهشی وزارت بهداشت)

INTRODUCTION: Fars is a disaster-prone province which is affected by a myriad of disasters, such as floods, earthquakes, fires, and traffic accidents. Therefore, the present study aimed to evaluate the disaster risk of hospitals in Fars province in 2017. METHODS: This descriptive cross-sectional study was performed in 2017. Out of 90 hospitals in Fars province, 51 centers cooperated in the presented study. Six prevalent disasters and crises in Fars province were identified using the recorded incidents in the last 20 years in the Emergency Operation Center (EOC) of Shiraz University of Medical Sciences. A book entitled "National tools for the assessment of risks and indicators of specialized competencies of the health sector in response to hazards and disasters (risk map), which was written for the Ministry of Health of Iran in 2014, was used to assess and obtain the risk score. The maximum and minimum risk scores were obtained at 95 and 19, respectively, and the data were analyzed in Excel 2016. FINDINGS: The mean and standard deviation of the obtained scores for different hazards were reported as 56.039±175.785 (earthquake), 45.962 ± 322.17 (seasonal flood), 575.786.195±19.57 (drought), 909.18±686.36 (chemical threats), 47.764±18.066 (human-caused fires), and 50.235±15.709 (power outage). In most hospitals, the risk of earthquakes and the negative impact of drought were higher than other hazards, while the chemical threats obtained the lowest score. CONCLUSION: Since Fars province is a disaster-prone area, the risk assessment should be periodically performed at short intervals to identify hazards with higher risk scores and implement corrective measures in this regard.

Challenges of Knowledge Management for Disaster Risk Reduction in Iran(مقاله پژوهشی وزارت بهداشت)

INTRODUCTION: Knowledge is a powerful resource to help governments, organizations, and communities to plan and improve their performance in mitigating the harmful effects of disasters. There is a crucial need for knowledge in all phases of disaster risk management, namely prevention, risk reduction, preparedness, as well as response and recovery. The present study aimed to assess the organizational, discourse, and metaphorical challenges and knowledge management improvement policies in disaster risk management in Iran. METHODS: This qualitative study was conducted in Iran in 2016. The needed data were collected through in-depth interviews with 21 experts and policymakers and analyzed by content analysis method. FINDINGS: Based on the obtained results, the following five categories were extracted: 1. current problems, 2. organizational and structural problems, 3. discourse level, 4. level of metaphors, and 5. knowledge management improvement policies in the disaster response phase in Iran.

Challenges and Needs of the Iranian Red Crescent Society in Response to the Great Flood of Iran in 2019(مقاله پژوهشی وزارت بهداشت)

INTRODUCTION: In the first half of April 2019, heavy rains in central and western parts of Iran resulted in great floods. The rains affected 24 provinces and caused extensive damage to urban and rural infrastructure. Since Iran, especially the northern, western, and southern provinces of Iran are prone to floods, the floods are expected to occur every year in many provinces. Therefore, the present study aimed to investigate the needs and problems of the Red Crescent Society in different provinces of Iran in response to the great flood of Iran in 2019. METHODS: This cross-sectional study was performed in all provinces of Iran after the end of the flood response operations in April 2017. The required data were collected through a researcher-made questionnaire and interviews with provincial operational managers. The designed questionnaire had different parts about the situation of the provinces in the face of floods, the most important problems faced during floods, and the most important needs for facing floods. Participants were also asked two questions about the information and equipment they should have had before the flood. FINDINGS: Analysis of the answers revealed that 24 provinces (77.41% of the country) were affected by the floods. The most common problems were lack of equipment and low public awareness which were repeated by the subjects 24 and 17 times, respectively. Moreover, the need for specialized equipment and training courses were also the most important needs. CONCLUSION: Based on the findings, the Iranian Red Crescent Society faced many problems and needs during the disaster relief process in the 2019 flood which indicated the lack of a comprehensive plan for flood management in Iran. Therefore, policymakers and disaster managers must develop practical and localized guidelines for relief according to the conditions of Iran in order to achieve the ultimate goal of disaster management which is to reduce deaths, injuries, and financial damages.

Evaluation of the Disaster-related Information Process from the Perspective of Managers in the Iranian Red Crescent Society(مقاله پژوهشی وزارت بهداشت)

INTRODUCTION: Information is known as the main source of power over others. In this respect, the use of correct, accurate, and timely information in decision-making, planning, and other related issues can be effective in the outcome of activities and services provided by organizations. In times of crisis and disaster incidence, information plays a key and crucial role and affects various disaster response processes. This study was conducted to investigate the communication and information processes and information analysis in the Iranian Red Crescent Society at the time of disasters in 2019. METHODS: This qualitative study with a content analysis design was carried out on 17 experts and managers in the Iranian Red Crescent Society. The necessary data regarding the existing information processes were collected through performing interviews with the participants. The collected data from conducting and implementing the interviews were then coded in MAXQDA software using qualitative analysis method and subjected to analysis. FINDINGS: Based on the findings, the majority of the activities of this organization were carried out by holding meetings to make the necessary arrangements. Measures, such as public education and reports to journalists, media, and social networks were provided in the form of written instructions and protocols. It was revealed that such communication means as media, newspapers, 112 emergency SMS system, and Thuraya satellite network were used for information. The tool utilized in the information process in the Iranian Red Crescent Society was the Disaster Management Information System to record information and the history of crises. This system was located and used in the Control and Coordination Center, and recorded all documents related to the events, including photos, videos, and reports and information of rescuers.
